Records Requests

Medical, Imaging, Facility Billing Requests

Northeast Georgia Health System (NGHS) is the records custodian. 

To ensure the fastest service, all medical records, imaging records, and facility billing records must be requested directly from NGHS. Requests can be submitted via fax, mail, or email through their process.

Professional Fee Billing Records

All professional fee billing records must be submitted to MBMS, a third party billing company. 

To request professional fee billing records, please fax requests to 302-261-8368. 

Note: These records are billing statements for the radiologist charges only and do not include any other services, such as imaging scans, procedures, hospital or facility charges, equipment, or supplies.
